Please complete the Driving School Bond application. In addition to the completed application the bond company underwriters will need the following information for review:
1. Current Personal Financial Statement – all owners (10% + stockholders) / spouses – our sample form follows
2. Current Business Financial Statement – for all businesses in operation one year or more – must include balance sheet and profit and loss statement
The premium rate is 5% based on the bond amount charged annually.
