Sales Tax Bond Application needs to be completed and signed by all owners/stockholders who own 10% or more of company stock as well as spouses if married in order to obtain approval for the referenced bond request. In addition to a completed application we will need the following information to underwrite your request:
1) Copy of Deposit Notice Requirement from the Board of Equalization
2) Current Personal-Financial-Statement of all owners/spouses-our sample form follows or you may use an existing statement prepared by a CPA
3) Current Business Financial if in business for one year or more – **if less than one year an opening balance sheet.
The sales tax bond amount will depend on what the taxing agency is requiring. The permium amount is betwee 5% and 15% of the bond face amount.
